Request records from any Hancock Health provider by downloading the form below or logging into our Patient Portal. You can mail or fax us your completed form. We can mail your requested records to you or fax them to other healthcare facilities. Please contact us with questions.

If you need care and can’t wait for an appointment, we have two convenient locations to serve you. Walk into an immediate care center for small cuts, aches and pains, cold and flu symptoms, or minor injuries.
