Long-Term Care
Patients who are critically ill or have complicated medical needs often require long-term hospitalization. They require close monitoring—more than can be provided with at-home or nursing home care. For these patients, Hancock Health provides long-term acute care services in partnership with AMG Specialty Hospital.
When Do I Need It?
Long-term care is for patients who need to stay in the hospital for more than 25 days before going back home. These patients often come from intensive care units. AMG runs special hospitals called long-term acute care hospitals (LTACHs) all over the country. LTACHs specialize in long-term care, which helps patients recover faster and saves money by reducing short-term hospital visits.
Hancock Health provides long-term care for the following conditions:
- Respiratory failure
- Ventilator dependence
- Post-operative complications
- Complicated infections
- Chronic non-healing wounds
- Cardiac-related disorders
- Closed-head injuries
- Infectious disease
- Septicemia
- Stroke
Referral & Admission Process
To be admitted to our long-term care unit, patients must have medical conditions that are clearly documented and require daily monitoring and/or intervention by healthcare professionals. If you are referred to us, an admission coordinator will schedule an onsite assessment. A clinical liaison will then confer with you, your healthcare provider, and your referring physician to document your goals and needs.
