About 70% of Americans suffer from high blood pressure at some point in their lives. Of this population, only about 1 in 4 people have their high blood pressure under control. Many turn to medications, but we are learning more and more that lifestyle changes can be even longer lasting when it comes to healthy blood pressure. That is why Hancock Health developed the Self-Monitored Blood Pressure Program.
The Self Monitored Blood Pressure Program is designed to help people lower their blood pressure without medication
Through educational, individual and group settings, this FREE program is tailored to each person’s specific needs. It provides valuable resources and information about how nutrition, physical activity and stress management can transform health. Other benefits include:
- Blood pressure cuff provided
- Individual blood pressure monitoring over the course of the program
- Four virtual, shared medical appointments (one every two weeks)
- Virtual or in-person, individual follow-ups at 3-, 6- and 12-month intervals
- Unlimited FREE 1:1 coaching and support from certified health coaches
Who qualifies?
To qualify for the Self-Monitored Blood Pressure Program, participants must meet two requirements. These include:
- Having a blood pressure screening of 140/90 or higher
- Or if you have recently been placed on blood pressure medications, or blood pressure medications have been suggested

Step 1: Reach out to Melissa Rumple, Clinical Coordinator, Chronic Care Management for Hancock Health, by filling out this form.
Step 2: One of our healthcare professionals will be in contact to review program details and the upcoming schedule of group sessions.
Step 3: You will schedule a time to come into our clinic to pick up your free blood pressure cuff monitor. You will meet one of our healthcare professionals who will go over paperwork, share a video, show you how to use the blood pressure cuff, and answer any questions you may have at that time. Then, you will receive an email invitation for the first virtual group visit.
Step 4: Group sessions will meet every other week for four sessions. This will be a rolling enrollment, allowing you to join when it is convenient for you. Once the four sessions are completed, virtual or in-person individual follow ups will be scheduled at 3, 6, and 12-month intervals. At the completion of the program, you may return the blood pressure cuff, or keep for ongoing use to monitor your blood pressure at home.
