Whole-Person Specialty Care enhances your care by providing quicker access to services that you may have previously had trouble accessing or were not covered by your insurance.
What is the Whole-Person Specialty Care Approach?
A whole-person approach involves focusing on a patient’s whole being. AndHealth’s care team of doctors, registered dietitians, rheumatologists, clinical pharmacists, and other specialty care providers work together with your Hancock Health provider. The AndHealth team tailors a treatment plan to uncover the hidden, underlying causes of your specific illness leading to long-term resolutions. Examples of underlying autoimmune disease symptoms may include:
-Food sensitivities
-Infections
-Blood sugar
-Stress
-Medications
-Gut imbalance (Dysbiosis)
-Leaky gut
-Nutritional deficiencies
-Hormones
-Environmental Exposures
The clinical team works to understand “you”, and the root cause of pain and swelling, and the complex, individualized nature of your symptoms. Through a comprehensive panel of labs, your AndHealth provider considers all factors of your health before ever creating a treatment plan.
What does Whole-Person Specialty Care include?
- Personalized Care: Participants can access nutritional and behavioral support, food delivery, health coaching, remote monitoring devices, and a dedicated app for easy communication with physicians and other providers (Program Care Team). Treatments are tailored to identify and address the underlying cause of your autoimmune condition and improve your health outcomes.
- No Additional Costs: This program is covered at no additional cost to you.
- Collaboration with Your Current Care Team: At your request, the Program Care Team will collaborate with others on your care team (e.g., your primary care doctor, another specialist, etc.).
- Concierge Specialty Medication Support: You will receive education about any medications you are prescribed, regular monitoring of the effectiveness and side effects of your medication, timely dose adjustments, guidance on self-administration, prior-authorization and co-payment assistance support, and medication delivery.
- Quick Access to Appointments: Wait times for appointments are usually less than two weeks. You can also chat with your Program Care Team through a secure app whenever you need to, without waiting for an appointment. There is no limit on how often you can see your Program Care Team.
- Virtual Provider Visits: Patients with a smartphone, iPad or a computer can opt for remote appointments with Whole-Person Specialty Care Providers from the comfort of their home, removing the need for waiting rooms or travel. Otherwise, virtual appointments can also be arranged at the Community Health Center.
- Transportation Assistance: If you would like to go to the Community Health Center for services or appointments, transportation aid is available.
If you do not believe you qualify for the Comprehensive Disease Management Program (CDM) or that the program aligns with your needs, Hancock is now also offering new rheumatology specialty care services to support your chronic care needs.
