Our Hancock Health Connection Center creates pathways to prevention, intervention, treatment, and recovery services. Our team of support navigators provide a confidential listening ear and help connect patients to local resources to overcome life’s challenges. We strive to provide a warm hand-off to the right services and remove barriers. Together, we can get healthy, stay healthy, live a healthier life, and thrive as a community.
